The truth is far more nuanced.

Cholesterol isn’t just some dietary demon; it’s a cornerstone of human physiology. Your liver produces around 700–1,000 mg of it daily on purpose, far more than most people consume from food. Far from optional, cholesterol is required for life. Here’s what it actually does.

The Architect of Your Brain and Nerves
Roughly 20–25% of your body’s total cholesterol resides in the brain, despite the brain making up only about 2% of body weight. It is essential for forming synapses (the connections between neurons) and maintaining the myelin sheath—the insulating layer around nerves that allows electrical signals to fire rapidly and efficiently. Without adequate cholesterol, nerve function falters.

The Foundation of Your Hormones
Cholesterol serves as the raw material for all steroid hormones. This includes:

Sex hormones like testosterone and estrogen.
Stress response hormone cortisol, which helps regulate metabolism, immune function, and your body’s reaction to challenges.
It also contributes to the pathway for vitamin D production in the skin upon sunlight exposure—a hormone critical for bone health, immunity, and mood.

Digestion and Cellular Repair

Why the Villain Narrative Persists
The focus on cholesterol as harmful grew from associations between elevated blood levels (particularly LDL) and heart disease risk in certain populations. However, this overlooks critical distinctions: the difference between LDL and HDL ("good" cholesterol that helps clear excess), the role of inflammation and oxidation in plaque formation, and the fact that dietary cholesterol has only a modest impact on blood levels for most people (your body tightly regulates production).
Modern research continues to affirm cholesterol’s necessity. Depleting it too aggressively—especially in older adults or those with specific metabolic profiles—has been linked in some studies to concerns around cognitive function, hormone balance, and overall vitality, though benefits for high-risk cardiovascular patients remain established in guidelines.
A Balanced View
This isn’t a call to ignore heart health. Lifestyle factors like diet quality, exercise, blood sugar control, and inflammation management matter immensely. Some individuals with genetic conditions or very high risk do benefit from targeted interventions. But for many, the blanket "lower is always better" approach deserves scrutiny.
